INFORMATION FOR THE COACH / TEAM MANAGER / PLAYERS
• All players must be within the proper age grouping as determined by USYSA rules.

• Teams will be required to present valid USYSA player passes, notarized medical
release forms, permission to travel forms, and certified state rosters at registration.

• No more than 3 guest players per team.

ACCEPTANCES AND REJECTIONS
Will be e-mailed by Friday, April 11; checks will be returned for teams that cannot be accommodated.

REGISTRATION
Registration will be held Friday, April 25 from 6:00 PM to 9:00 PM at the tournament headquarters which is the Park Inn - 909 HWY 70 SW, Hickory, NC 28602 (I-40 Exit 123, Exit 44 off Hwy 321). Each team is required to present valid USYSA player passes, notarized medical release forms, permission to travel forms and certified state rosters at registration.

for reservations.

APPLICATION INFORMATION ENTRY FEE
Entry fee is $475. Make checks payable to “Rotary Club of Lake Hickory” and include with application.

ROSTER
Please include a copy of your team's certified state roster with your application.

"Classification": Please indicate whether your team is Premiere, Classic, Challenge, or Division 1, 2, or 3.

APPLICATION DEADLINE
